Q. WHAT SHOULD I DO IF AN ON-THE-JOB INJURY OCCURS?

1. Obtain medical treatment, using the preferred provider list. If a preferred provider is not listed in your area, seek treatment at the nearest medical facility. If you would like to suggest a medical provider in your area, contact CCM, Inc. Utilization Dept. at

2. Prepare the WC2-Employer's First Report of Injury form (you received one in your renewal packet) and Mail or Fax this form to CCM.
