Injured? Who to Call

If an on-the-job injury occurs, obtain medical treatment using the Preferred Provider Organization (PPO) list below. If a preferred provider is not listed in your area, seek treatment at the nearest medical facility. If you would like to suggest a medical provider in your area, contact the CCM, Inc. utilization dept.
